You have the right to receive a “Good Faith Estimate” of expected charges under the No Surprises Act.

• You will receive a written estimate of session fees before your first appointment.
  
  

• The estimate will include the cost of services we discuss and your expected out-of-pocket expenses.
  
  

• This estimate is not a contract, and your actual number of sessions or total cost may vary depending on your needs and goals.
  
  

• If your bill is at least $400 more than your Good Faith Estimate, you can dispute it.

Your privacy is important. Rooted Oak Family Therapy complies with HIPAA standards to protect your health information.

This means:

• Your records are kept secure and confidential.
  
  

• Information is only shared with your written permission, unless required by law (for example, safety concerns or court order).
  
  

• You have the right to request copies of your records or ask for corrections.

Your appointment time is reserved just for you. I understand that life happens and schedules sometimes change. If you need to cancel or reschedule, please let me know at least 24 hours in advance whenever possible.

• If we can find a new time within the same calendar week, there is no cancellation fee.
  
  

• Same-day cancellations, missed appointments (no-shows), or arriving more than 15 minutes late without notice will be charged a $100 fee. If you and I agree to continue with the remaining time after a late arrival, the full session fee still applies.
  
  

• If I have your card on file, the fee will be charged automatically. If not, it will need to be paid before our next session.
  
  

• Cancellations or reschedules initiated by me will never incur a fee. In cases of inclement weather, we can switch to telehealth, reschedule, or cancel without penalty.
